1...Send a filled in copy of this form to CCF (84-90 Chipstead Valley Road, Coulsdon, Surrey. CR5 3BA) with a cheque payable to CCF 
2......................Copy this form onto an email.  Fill it in.  Email it (chess@ccfworld.com).  Then send a cheque to CCF with a covering note.
3................................................................................Phone CCF and pay by credit card, given these details over the phone or by email.

Entering for between 20 and £30 players

Above 30 players

£35 + add £5 for every extra 5 players (or part thereof)
Anyone qualifying for the finals will have their name and school on the CCF web site.  Anyone playing in the qualification events does so under this condition and we shall assume all schools have confirmed with parents/carers that this is acceptable.
